When you should consult a gynecologist for infertility treatment

Ovulation Induction with Timed Intercourse- In this case, the woman takes certain ovulation induction medications in the form of tablets and injections from the prescribed days. It stimulates the follicles (egg bags ) to grow. Subsequently, follicular studies are done on alternate days to know the size of the leading follicles. Later, HCG injections are given that trigger follicles to rupture and release the eggs. At this time, the couples are advised to have timed intercourse for a successful conception.

Intra-Uterine Insemination (IUI)- In this procedure, after ovulation induction is done, around the time when the follicles rupture, sperms are inserted inside the woman’s uterus to facilitate fertilization. This process increases the number of sperms reaching the fallopian tubes, which in turn increases the chance of fertilization and conception.

In-vitro Fertilization (IVF)– Invito Fertilisation or IVF is an advanced medical procedure which improves the pregnancy rates greatly.

In IVF, eggs are fertilized under in-vitro conditions. When the egg develops into an embryo, it is transferred into the uterus of the woman. In general, it is required when there are defects in the fallopian tube, or the count of sperms is low. The following are the important steps involved in IVF procedure:

  • Step 1- Starting Ovarian Stimulation : When you get your cycles, on the second day of your period, you will undergo an ultrasound scan and blood test to see how your ovaries are. And then, you will be given hormone injections , either Follicle Stimulating Hormone (FSH) or Human Menopausal Gonadotropins ( HMG) to stimulate the follicle growth. These injections will last for almost 10 days.
  • Step 2- Follicle monitoring: During this period of ovarian stimulation, your follicle growth will be monitored through ultrasonography and blood tests.
  • Step 3 – Follicle triggering : When the follicles achieve the >17mm size, egg maturation will be triggered by giving HCG /Leuprolide injection. Egg recovery will be booked following 36 hours of the trigger.
  • Step 4- Egg retrieval: Egg recovery is arranged under general anesthesia, using transvaginal ultrasound guided oocyte recovery needle. Eggs are delicately retrieved from the follicle and sent to the Embryology lab for evaluation of oocytes. After the procedure, you will be observed for couple of hours and then sent home. Light spotting and stomach spasms are normal after egg retrieval.
  • Step 5- Fertilization: The eggs retrieved are examined and mature eggs are selected. The good sperms selected from the male partner after washing and preparing the semen is used for fertilising the develop oocytes. The fertilised eggs are carefully cultured in controlled incubators for 3-5 days and periodically checked for their development.
  • Step 6- Embryo transfer: The fertilized eggs , now called embryos are ready for transfer into the womb. They can be tranfered either in the same cycle or in the next menstrual cycle (when its called frozen embryo transfer). Usually either one or two embryos are transferred into the womb. This is a simple out-patient procedure and doesn't require any anesthesia. After embryo transfer you will are advised rest for a couple of hours and then sent home.

Blastocyst Culture

Blastocyst is an embryo that has developed for five days after fertilization. Previously, day 3 embryos were routinely transferred. Now, with improvements in the scientific technology and embryo culture is possible till day 5, provided the eggs and sperms are of good quality.

  • Better selection of the embryos to be transferred
  • Limit the number of embryos to be transferred
  • Eliminate the risk of multiple pregnancy
  • Better implantation rate
  • Step 7 - Luteal support: This is started soon after your egg retrieval is done, progesterone injections or tablets are given daily and estrogen supplements started to prepare the uterus to receive the pregnancy. Post embryo transfer also this support is continued till the pregnancy test is done.
  • Step 8 - Pregnancy test and follow up: After 15 days of embryo transfer, pregnancy test is done, this will usually be a blood test, if the test is positive the progesterone support is continued for a few more weeks.

Intracytoplasmic sperm injection (ICSI)

This is a more extensive form of IVF wherein sperm is injected into the egg with a special needle under microscope.
